Extra Care housing aims to meet the needs and aspirations of Individuals through the provision of Services designed to accommodate changing needs, for example allowing older people to age in one place and preventing avoidable and/or emergency hospital or residential care admissions.
Typically, Extra Care housing provides a supportive environment, offering a range of Services, from unplanned low-level care and physical response including night-time cover as well as individually tailored care Services (person-centred Commissioned Care). The Service has been developed to provide integrated health and social care Services for people living in Extra Care and those assessed as eligible for care.
